Ballot
Ballot is a document used to register shareholders' votes when a company shareholders vote on company issues. It is usually just a piece of paper with possible choices printed, and every shareholder who attends annual meeting is required to fill it.
Ballot could also be in electronic form, or even supplied trough a phone line, if the shareholders is unable to attend the annual meeting in person.
